Wellness House is a functional wellness and integrative health consulting practice. Our services are centered on nutrition, lifestyle education, and foundational wellness support. We do not provide medical diagnosis, medical treatment, or disease management. Wellness House does not claim to diagnose, treat, cure, or prevent any disease or medical condition.

All healthcare decisions, diagnoses, and medical treatments must be made in consultation with your licensed medical provider.

Although members of our team may hold professional licenses (including naturopathic physicians and registered dietitians) in certain jurisdictions, all services rendered through Wellness House are offered strictly in an educational and consultative capacity. Practitioners are not acting as licensed medical providers within the scope of Wellness House services.

Client wishes to retain the services and advice of Wellness House LLC for the purposes of nutrition and holistic health management services.

Client hereby acknowledges and agrees to the following agreement, including terms and conditions regarding nutritional and holistic health counseling services provided by Wellness House:

1. Services

Client understands and acknowledges that Wellness House offers nutritional and holistic health counseling services through its team of nutritional practitioners, and its clinical team of naturopathic health advisors (“Services”). In connection with such Services, Wellness House agrees to refer Client to third-party advisors (“Advisors”) of the Wellness House network, and that such Advisors are not employees of Wellness House, but rather, independent contractors operating with the referral of Wellness House. Client understands that the nutritional counseling services provided by Wellness House focus on wellness and vitality through non-toxic, natural nutritional therapies. While embracing a healthier attitude, lifestyle, and diet can generally contribute to greater health and wellness, Wellness House does not promise or guarantee protection from future illness. Client understands and acknowledges that Wellness House may request certain third-party laboratory tests as a part of the selected Services plan and may use such tests to review the nutritional requirements of the Client. Such test results should be reviewed by a licensed medical doctor and are not used as a medical diagnosis nor medical advice. If Client suspects that he/she has a medical condition requiring medical attention, Client agrees and understands that Client shall independently consult with a licensed physician immediately.

2. Financing Plan and Payment

Client acknowledges and agrees to enter a financing plan, and such financing plan is subject to the terms and conditions of the Financing Agreement (if utilizing Affirm please see your Affirm loan confirmation for details). Client further acknowledges the Services provided herein are contingent upon payment and execution of the Financing Agreement. Such payment for Services provided by Wellness House is due and earned at the time this Agreement is executed regardless of the Client’s election to terminate or use of such Services. Such Financing Agreement along with any payment made to Wellness House are non-refundable upon the execution of this Agreement.

3. Disclaimer

Client agrees to have all suggestions, dietary changes, and nutritional supplements recommended by Wellness House approved by Client’s licensed medical doctor. Client understands and acknowledges that Wellness House is not providing medical diagnosis or treatment. All decisions regarding Client’s health and well-being should be made in consultation with a licensed medical professional. Client acknowledges that it is Client’s responsibility to independently seek the advice and diagnosis of a licensed medical doctor for any medical conditions or illnesses. Client further acknowledges that as a part of Services herein, Wellness House refers its clients to third-party contractors and professionals, including medical doctors, nutritionists, laboratories, phlebotomists, counselors, and other professionals who are not within Wellness House direct control. Only a licensed physician can provide medical treatment and diagnosis. Wellness House, its naturopathic physicians, and nutritionists do not treat conditions or prescribe medication.

4. Naturopathic Physician and Registered Dietitians Disclaimer

Wellness House provides consultation-based services focused on education and wellness support. These services do not constitute medical care and are not a replacement for individualized medical care provided by a licensed healthcare provider.

Wellness House works with naturopathic physicians and registered dietitians who maintain active licensure in their respective jurisdictions; however, in the context of our virtual consultation services, they are not acting as licensed medical providers. They are acting strictly as consultants, providing education and information.

All information, suggestions, dietary or lifestyle changes, supplement recommendations, or educational materials provided through Wellness House are for informational and educational purposes only. They do not constitute medical advice, diagnosis, or treatment and should not be relied upon as a substitute for individualized medical care.

Any recommendations you receive through Wellness House should be reviewed with and approved by your licensed healthcare provider prior to initiation. By participating in these services, you acknowledge that you are not establishing a doctor–patient relationship with any of Wellness House’s naturopathic physicians. They cannot diagnose, treat, or prescribe medications. You are required to maintain a relationship with a licensed primary care provider who remains responsible for your medical care. Clients are solely responsible for consulting with a licensed medical doctor or other qualified healthcare professional regarding any questions, health conditions, medications, or treatment decisions. Wellness House does not provide medical care and is not liable for any decisions or outcomes arising from the use of its services or from any third-party professionals to whom clients may be referred.

5. Laboratory and Test Kits

In the event of labs or test kits, Wellness House strives to make such a process as private, safe, and efficient as possible. Nevertheless, Client acknowledges that the shipping and handling of such kits may, from time to time, become lost or stolen because of events outside of Wellness House’s control. Client understands and agrees that Wellness House shall not be liable or responsible for any lost or stolen mail when shipping lab test kits, including USPS, FedEx, UPS, et al. Client agrees that he/she will notify Wellness House as soon as possible if Client believes such test kits have been lost or stolen. Client has provided his/her current mailing address and will notify Wellness House as soon as possible if there are any changes to Client’s mailing address. Client understands and acknowledges that in certain states (including, without limitation New York, New Jersey, Rhode Island), various lab tests are restricted by mail. In such cases, Client acknowledges and consents that any such tests or laboratory requirements will be conducted in compliance with applicable law or regulation. Client shall not be entitled to a refund and agrees to comply with all applicable laws, regulations, or legal processes.

6. Contacts

Any concerns or questions regarding payment or billing should be directed to info@wellnesshouse.io(or your preferred admin email). Client understands that Wellness House does not offer refunds. Client agrees to pay the full amount for the Services selected. If there are changes to Client’s credit or debit card information, Client agrees to notify Wellness House at least 48 hours before a scheduled payment is due (not applicable if using Affirm).

7. Assumption of Risk

Client assumes the risk and acknowledges that any therapies or services undertaken through Wellness House are voluntary and at Client’s own risk. Client accepts that ultimate responsibility for healthcare lies with Client, and Wellness House provides support only. The consultation services provided by Wellness House, including nutritionists and naturopathic consultants, are not a substitute for medical treatment or diagnosis of specific illnesses or disorders.

8. Release of Liability

Client waives and releases Wellness House, its affiliates, and their partners, members, shareholders, employees, contractors, and representatives from any and all claims or causes of action arising from or connected with the Services rendered by Wellness House.

9. Prior Illness and COVID-19

Client agrees to disclose to Wellness House any ailments, chronic illnesses, or contagious conditions and to seek medical care as appropriate.

10. Termination by Wellness House

Wellness House reserves the right to terminate services at any point. Any remaining unused services will be refunded within 10 business days. Upon refund, Client releases and discharges Wellness House and its Representatives from all claims.

11. Confidentiality

All consultations, methods, and programs offered by Wellness House are confidential. Breach may result in termination without refund.

12. HIPAA Compliance

Wellness House will maintain privacy of protected health information as required by law.

13. Social Media and Public Disclosure

Client agrees not to share or publicly disclose information regarding Wellness House without prior written consent.

Release and Waiver of Liability for Wellness House LLC

I consent to my Wellness House practitioner recording my individual 1:1 sessions for quality and training purposes. Recordings are confidential and internal only.
